How to get a medical card in Nevada

Joining the Program

In this section of the site we’ll provide you with the knowledge and tools you’ll need to move forward with becoming a registered patient.  We’ll give you with some background on the Nevada medical cannabis program, detail the list of qualifying conditions and how/why cannabis can be helpful for each condition, walk you through every element of the application process, provide interactive maps showing you where your nearest fingerprinting facility is located and more.

Obtain a form to request an application from the state.

Fill out the above request form.

Send the request for an application to the state and $25 (check written to Division of Public and Behavioral health DPBH.)

You will then receive an application from the Department of Public and Behavioral Health.

On this application:
a) Information about applicant filled out and signed by applicant
b) Attending Physician’s Statement – Filled out by applicant’s doctor (Must be Board certified Medical Doctor or Osteopathic Doctor in State of Nevada, in good standing).
c) Waiver and Acknowledgment – Both forms must be notarized.
d) Caregiver Application – If requested by applicant or if the applicant is a minor
e) Minor Release Form – Required if applicant is under 18 years and must be notarized (Applicant responsible for notary fees)
f) Proof of Nevada Residency – A copy of the applicant’s driver’s license or Identification card issued by Nevada DMV must be returned with required forms

Send back the application

Send back the application with $75 payable to the Department of Public & Behavioral Health with the Physician’s statement, the waiver & acknowledgement (notarized) and a copy of your Nevada Driver License.

DPBH processing

The DPBH will process the application (checking to see if there are no convictions for knowingly or intentionally selling a controlled substance).

Acceptance Letter

Approved applicants receive an acceptance letter that must be taken to an approved NV DMV location to get their Medical Marijuana Patient card. The cost of Registry photo ID card is $11.

Congratulations!

You are on the road to good health. Once the DPBH receives the application, it should take 30 days.

Approved Conditions for Medical Marijuana in Nevada

Acquired immune deficiency syndrome (AIDS)
Cancer
Glaucoma
Post-traumatic stress disorder (PTSD)
Cachexia (general physical wasting and malnutrition from chronic disease)
Persistent muscle spasms (including multiple sclerosis)
Seizures (including epilepsy)
Severe nausea
Severe pain (the most commonly reported condition)
